Play Therapy Documentation Made Simple

Play Therapy Documentation Made Simple

Documentation can feel overwhelming, especially when you’re trying to make your paperwork reflect the depth and complexity of play therapy.

Play Therapy Documentation Made Simple will help you see documentation differently: not as a collection of disconnected forms, but as one connected clinical process that tells the story of the child from intake through outcomes.

In this practical training, you’ll learn how to connect your parent questionnaire, intake, diagnosis and medical necessity, treatment plan, progress notes, andC outcomes so that each piece of documentation builds on the one before it.

And you won’t just learn what to document, you’ll leave with ready-to-use, play therapy-specific templates, including:

  • In-depth Parent Questionnaires designed to gather the clinical history and information you need to truly understand the child before treatment begins.

  • Treatment Planning Templates with play therapy-specific guidance for documenting medical necessity and developing meaningful treatment goals with language you can copy and paste directly into your treatment plans.

  • Play Therapy Progress Note Templates designed to capture what matters in play therapy, including the stage of therapy, themes, therapeutic experiences, and clinical meaning of the child’s play.

  • A clear framework for connecting everything from intake → diagnosis & medical necessity → treatment planning → case notes → progress & outcomes.

You’ll walk away with a documentation system that is clearer, more intentional, and actually useful in your clinical work without having to reinvent your paperwork for every child.

The goal isn’t to document more.
The goal is to document what matters.

By the end of this training, participants will be able to:

  1. Describe how the major components of play therapy documentation connect to create a cohesive clinical story from intake through treatment outcomes.

  2. Identify key clinical information to gather through a comprehensive parent questionnaire and intake process to support assessment and treatment planning.

  3. Explain how to document diagnosis and medical necessity in a way that connects presenting concerns, functional impairment, clinical needs, and the use of play therapy.

  4. Develop play therapy treatment goals and objectives that are clinically meaningful, measurable, and aligned with the child’s identified needs.

  5. Apply a play therapy-specific framework for documenting sessions, including the stage of therapy, themes, therapeutic experiences, and clinical meaning of the child’s play.

  6. Utilize play therapy-specific documentation templates to create more efficient, consistent, and clinically connected parent questionnaires, treatment plans, and progress notes.

This course is eligible for 3 APT Approved Non-Contact CE hours.
